IHI shares new tool for end-of-life care conversations

"Conversation Ready" Respect Tool [+]

End-of-life care conversations aren’t always easy, but they are necessary. And they are increasingly relevant as patients with serious conditions live longer and are able to express their care preferences. "Conversation Ready": A Framework for Improving End-of-Life Care (2nd edition), a white paper from the Institute for Healthcare Improvement, offers background, guiding principles and tools to help health care organizations engage patients and families in advance care planning.

The Respect Measurement Tool allows an organization to assess its performance on six elements of respectful engagement with patients and families as they develop their care plans. It also promotes reflection and conversation about care of individual patients from different perspectives.
